This form should be used to file a whistleblower complaint regarding: The F.W.S. Office Supervisor, An Officer of F.W.S. (President, Treasurer or Secretary), Designated non-BOT members of the Board HR/Personnel and Board Finance Committees, Any Board of Trustees Member.
This form should NOT be used to file a complaint about an indvidual, an intergroup, or individual representative unless they are an F.W.S. trusted servant.
F.W.S. has third-party whistleblowers who will review this submission. Neither a Board member, nor an administrator of F.W.S. will receive this complaint upon submission. If you do not hear back within 10 days, you may contact the F.W.S. webmaster at [email protected] and let them know that you submitted a complaint. Do not include any details in your message, just that you submitted a complaint, and the webmaster will run a script that will resend any complaints received within the past 10 days to the whistleblowers. Complaints are permanently deleted from the webserver after 12 days.
