Whistleblower Complaints

This form should be used to file a whistleblower complaint regarding: The F.W.S. Office Supervisor, An Officer of F.W.S. (President, Treasurer or Secretary), Designated non-BOT members of the Board HR/Personnel and Board Finance Committees, Any Board of Trustees Member.

This form should NOT be used to file a complaint about an intergroup or individual meeting representative.

F.W.S. has third-party whistleblowers who will review this submission. Neither a Board member, nor an administrator of F.W.S. will receive this complaint upon submission.

Because this form is hosted on the F.W.S. website, the F.W.S. webmaster may have access to the form submission for up to 14 days from submission while its contents are held in our records before deleting. Whistelblower complaints regarding the webmaster may be sent to BOTChair@slaafws.org to bypass this form.
